Workers' Compensation

The San Bernardino County Superintendent of Schools participates in the California State Workers' Compensation program. Risk Management Services is responsible for implementing this program. Our Worker's Compensation Insurance Provider is Your Insurance Services group

Every employee who suffers an injury, no matter how slight, must immediately notify his/her immediate supervisor and Risk Management Services.

What to do if you are injured on the job
  1. Report the injury to your immediate supervisor or his/her designee and then call Risk Management Services at (909) 386-2417. Risk Management Services will request some basic information and then transfer you to the Company Nurse Injury Hotline.
  2. If Risk Management Services is unavailable by phone or if you are out of town on official SBCSS business and are injured, you may contact the Company Nurse Injury Hotline directly at (888) 375-0280.   3. All injured employees will be referred to the Company Nurse Injury Hotline for triage by a nurse or a referral to one of our clinics. The Company Nurse Injury Hotline will send all authorizations to the clinic by fax. All injuries must be reported, even those that do not require medical attention by a physician.
  4. Employees who have predesignated a personal physician are told to see their own doctor after first notifying Risk Management Services. Risk Management Services will need to verify that a completed Predesignation of Personal Physician form is on file for the employee and then fax an authorization to the physician.

  5. The employee and supervisor will be required to complete an “Employee-Supervisor Incident Report” and submit it to Risk Management Services within 72 hours of receipt. The incident report will be sent to the employee and supervisor by Risk Management Services and is required for all injuries.
  6. In some cases, the injured employee will also receive a Workers’ Compensation Claim Form (DWC-1) in addition to the Employee-Supervisor Incident Report. These forms are then forwarded to our claims adjuster for further processing. All forms need to be returned to Risk Management Services within 72 hours of receipt.
